Prescription refills: Your prescription can be refilled in most instances by calling your pharmacy Monday through Friday. An electronic request will be sent to our office from the pharmacy. We do not refill routine prescriptions on the weekend unless it is an emergency. Our office policy prohibits the filling of narcotic prescriptions on the weekend. Due to the large volume of prescription requests, the office has a 72-hour turn around time for completing a prescription refill request. Please plan accordingly.
We ask you to bring your prescription bottles with you to your appointments. This gives our staff the opportunity to check all your medications and make sure that you are taking only appropriate medications and to review for timely refills. Our office reserves the right to ask you to come in for an appointment if we feel it is necessary.
Preventative Health: As Family Physicians we are concerned with promoting wellness. We encourage regular annual physical examinations and preventative health screening test. Please feel free to discuss these recommendations with your primary healthcare provider.
Insurance: We have contracts with a wide variety of insurance companies. It is your responsibility to know the details about your specific insurance coverage. It is also your responsibility to notify our office of any changes to your insurance. Please bring your current insurance card with you to each and every visit.
Payment Policy: If your insurance company requires a co-payment for office visits, you should be prepared to pay this fee at the time of the visit. (Our office has a $10.00 charge for completion and mailing of bills for co-payments that are not paid at the time of service.) Your co-payment is a portion of the insurance reimbursement and waiving this fee is in violation of the contracts with the insurers. Therefore our office does not waive co-payments except on a case by case basis for severe financial hardship. In the event of hardship, our office will request documentation for the medical record.
All charges that are the responsibility of the patient are due at the time of service. Payment plans apply to old balances only.
Confidentiality: We treat your medical information with the utmost respect. Occasionally this may seem burdensome for you our patient, however it is to protect your healthcare information. Please refer to our Notice of Privacy Policy for additional explanations.
Letters and Forms Policy: Our office will gladly complete forms and or letters that you may need from time to time for disability insurance, physical information, etc. There are certain conditions to providing this service. First, we must have clear documentation of the requested information within your medical record. Secondly, completion of forms and letters is not a priority. Our physicians, nurse practitioners and physician assistant provide care to our patients first. Completion of forms and letters is done as time allows; therefore it may take up to 10 business days for forms to be completed. In some cases charges may apply. Please plan accordingly.
Call backs: We will return your phone call in most cases within 24 hours. Occasionally, due to circumstances beyond our control, such as the provider being called out of the office for an emergency, you may not receive a call back within 24 hours. Please be aware due to privacy concerns we will not leave a detailed message on an answering machine unless we are confident that it is the correct answering machine and you have given us permission to leave a message on that machine when you initiated your call.
Medical Records: Our office will provide one copy of your medical record as a courtesy if you transfer care from our practice. In most other circumstances (disability cases excepted) there is a fee for providing copies from your record. This is a nominal charge and is allowed by law. Copies will be provided only after the appropriate release form is complete. This service is completed as soon as reasonable.
Our Rights: As a private practice, we reserve the right to institute policies as necessary to conduct our medical practice business as we see fit. This includes the right to discharge patients due to various reasons. Some examples include, abusive behavior, non-compliance (repeated no shows for appointments), altering prescriptions and inappropriate narcotic seeking. In the event a patient is discharged, he/she may be able to continue to receive care for thirty days while securing a new healthcare provider.
